Medical Accommodations
The Briarcliff Manor Union Free School District is committed to providing reasonable accommodations to qualified employees with disabilities in accordance with the Americans with Disabilities Act of 1990, as amended (ADA).
Employees seeking a workplace accommodation should complete the Accommodation (Medical) Request form available in Frontline Central under My Forms and Forms I Can Start. Directions for completing the request are included within the online form.
The purpose of the form is to assist the District in determining whether a reasonable accommodation can be provided to support an employee in performing one or more essential functions of their position. By submitting the form, the employee authorizes the District to explore potential coverage and reasonable accommodations under the ADA.
All information submitted, including medical and genetic information, will be maintained in strict confidence and handled in accordance with the confidentiality requirements of the ADA and the Genetic Information Nondiscrimination Act (GINA).
If additional information or clarification is necessary to evaluate the request, the employee authorizes the Director of Human Resources or the District physician to communicate with the medical or mental health professional who provided the supporting documentation.
Upon completion by the medical provider, employees should upload the completed and signed documentation through the Frontline Central accommodation request form for review by the Director of Human Resources.
