Dear Parents/Guardians:
As per the New York State Education Law, students entering Grades 9 and 11 must submit a physical to the health office to attend school.
As per Public Health Law, students entering Grade 12 must submit a current immunization record reflecting they have received 2 doses of the Meningococcal Conjugate vaccine or 1 dose of the vaccine if the 1st dose was received at 16 years of age or older.
